Title:
Improve your teaching with formative evaluation
Details:

Formative Evaluation is a type of evaluation that occurs throughout the course. Its intent is to assess ongoing class activities and provide information to monitor and improve the class. It is done at several points in the developmental life of a course and its activities. According to evaluation theorist Bob Stake,

"When the cook tastes the soup, that's formative;
When the guests taste the soup, that's summative."

The following is an evaluation form used by Physical Therapy faculty.
